Zepbound and Wegovy Without Insurance: Dose and Supply Costs

The manufacturer prices, starter-dose restrictions and refill deadlines I would check when comparing Wegovy and Zepbound without insurance.

The short answer

Without insurance, the Wegovy pill starts at $149 a month and the Wegovy pen at $199 for two eligible0.25 mg/0.5 mg starter fills, then $349 for doses through 2.4 mg. Zepbound starts at $299 at the lowest dose and runs up to $449 or more at the higher ones. The price climbs with the dose, so the starting number is rarely the one you keep paying.

Why I made this table

I would separate list prices from the price an eligible self-pay patient is offered. I checked the manufacturer terms rather than assume a headline is the full ongoing bill1. Both makers now sell straight to patients with a prescription, and the government's TrumpRx site lists their cash prices too.

The trouble is that each site shows one headline number, and the number changes as you go up in dose. So I read all three pages and lined them up. These are the prices checked October 18, 2026.

For Wegovy injections, the listed box contains four weekly pens; Zepbound’s KwikPen supply is 28 days. Pill prices are for30 tablets. I would compare those supply units rather than assume every advertised month is a calendar month. Taxes, fees and program eligibility can affect the final charge.

Why the same drug has three prices

This is the part that confused me most. TrumpRx shows the Wegovy pen at $199 and Zepbound at $299, with no dose attached1. Read the makers' own pages and those turn out to be the bottom of each ladder.

For Wegovy, $199 applies to two eligible monthly fills of 0.25 mg or0.5 mg for new patients. After that the pen is $349, and the 7.2 mg dose is $3992.

For Zepbound, $299 is the 2.5 mg starting dose, which Lilly notes is not a maintenance dose. The higher KwikPen doses are $449 only while you keep refilling within 45 days of delivery or receipt of your previous KwikPen prescription. Miss that window and the regular price applies: $499 at 7.5 mg and $699 from 10 mg up3. That one rule could cost a forgetful month a few hundred dollars, so I would record the delivery date and ask how to preserve eligibility.

Pens, KwikPens and vials

Zepbound comes three ways, and the cheap self-pay price is not for all of them. Lilly says its $299 starting price is for the KwikPen or the vial, not the single-dose pen3. The KwikPen holds four doses in one pen5. Vials mean drawing up your own dose with a syringe, which requires measuring the prescribed amount rather than assuming pen instructions apply I would ask for the vial price at my prescribed dose rather than assume it matches the KwikPen.

If you are on Medicare

Medicare's GLP-1 Bridge charges a $50 monthly copay for Wegovy, Foundayo and the Zepbound KwikPen for people who qualify, from July 1, 2026 through December 31, 20274. Your prescriber has to confirm you meet its criteria. The $50 doesn't count toward your Part D deductible or yearly out-of-pocket cap. More on the coverage side is in getting a GLP-1 covered, and the Medicaid picture is in GLP-1 coverage, state by state.

Brand at your real dose vs compounded

I would compare the exact prescribed dose and supply, then add consultation, membership, labs and shipping where required. Compounded offers vary widely in term and inclusions; a low starting figure is not evidence of a typical ongoing price. I would not infer the cheaper option from a brand starter offer and a compounded multi-month equivalent.

Compounded is a different product with different oversight, which I wrote about in compounded semaglutide vs Ozempic. And the pills have their own trade-offs, in Foundayo vs the Wegovy pill. The compounded side of the cost question is in comparing GLP-1 costs without insurance.

What I would do before paying

  1. Ask the prescriber which future doses may be appropriate and price those separately; do not assume your month-four dose.
  2. Write down the date of every refill if you are on Zepbound, because the 45-day offer clock runs from delivery or receipt of the previous prescription.
  3. Check the maker's page the week you fill. Several of these prices have already changed this year, and the intro offers have end dates.

None of this is medical or financial advice. Prices are the companies' own and can change without notice. If a dose increase leaves you vomiting or unable to keep fluids down, call your prescriber before the next shot.

Frequently asked questions

What supply does Zepbound self-pay pricing cover?

The cited KwikPen prices cover a 28-day supply: $299 at 2.5 mg and $399 at 5 mg. Higher doses cost $449 under the offer when refill timing qualifies; regular prices are $499 at 7.5 mg and $699 at 10–15 mg. Confirm formulation, eligibility and current terms.

Who qualifies for the Wegovy $199 starter offer?

The cited offer is for new patients’ first two eligible monthly fills of the 0.25 mg or 0.5 mg injection, through December 31, 2026. It is not a price for every dose. The subsequent pen price is $349; 7.2 mg is $399. Confirm current terms.

How is Wegovy pill pricing different?

NovoCare lists $149 for 1.5 mg, $199 for 4 mg and $299 for 9 mg or 25 mg, per 30-tablet supply. Compare the prescribed dose and supply rather than a starting headline.

When does the Zepbound 45-day offer clock start?

The cited KwikPen offer measures the refill window from delivery or receipt of the previous prescription. Confirm the exact deadline and eligibility with the dispensing service.

Does the Medicare Bridge apply to everyone?

No. The temporary program has eligibility criteria and a $50 monthly copay for qualifying people from July 1, 2026 through December 31, 2027. The copay does not count toward the Part D deductible or annual out-of-pocket cap.
