Real moms, real GLP-1 talk

Foundayo vs the Wegovy Pill: The One Without the Morning Rule

The daily routines, contraception precautions and dose-specific costs I would compare before choosing an oral GLP-1, with limits on cross-trial results.

The short answer

Both pills start at $149 per listed 30-day supply for eligible self-pay patients. Foundayo is taken once a day with or without food, at whatever time suits you1. The Wegovy pill has to go down on an empty stomach in the morning with a little water, and then nothing else for 30 minutes2. The labels report different average losses in separate trials; those results do not establish which pill is more effective.

Why I went looking

I read both labels to compare the daily routine. The Wegovy pill discussion explains the morning requirements. I would ask how a thirty-minute wait fits with breakfast, other medicines and an unpredictable schedule before treating a tablet as automatically more convenient.

The food instruction is a practical difference. It is not the only difference: contraception, dose, tolerability and cost also need comparing.

The routine, side by side

RuleWegovy pillFoundayo
FoodEmpty stomach, in the morningWith or without food
WaterUp to 4 ounces, water onlyNo rule on the label
Wait before eating or other pillsAt least 30 minutesNo wait on the label
How oftenOnce a dayOnce a day
Birth-control pill warning on the labelI didn't find oneYes, for 30 days after starting and after each dose increase

The birth-control part surprised me. The Foundayo label says that if you use a birth-control pill, you should switch to a non-oral method or add a barrier method for 30 days after you start and for 30 days after every dose increase1. It says hormonal methods that aren't taken by mouth shouldn't be affected. If you are on the pill and very much done having babies, put that on your list before anything else.

What it costs, dose by dose

Both companies sell the pill straight to you if you have a prescription. These are the self-pay prices on their own pages checked October 18, 20263,4:

StepWegovy pillFoundayo
Starting dose$149 (1.5 mg)$149 (0.8 mg)
Second step$199 (4 mg)$149 (2.5 mg) for now, normally $199
Third step$299 (9 mg)$199 (5.5 mg) for now, normally $299
Higher doses$299 (25 mg)$299 (9 mg); $299 to $349 (14.5 and 17.2 mg)

Two catches on the Foundayo side. The lower prices on the second and third steps are a temporary cut that Lilly says runs through December 31, 2026. And the $299 at the two top doses depends on buying each refill within 45 days of delivery or receipt of the previous prescription; otherwise it is $3493.

So at the start they cost the same. Further up the ladder, which one is cheaper depends on where you land, and both companies have already changed these numbers this year. If you want every brand price in one place, including the shots, I put that in Zepbound and Wegovy without insurance.

Wegovy’s pill price is for a bottle of 30 tablets; Foundayo defines a month as 30 days. Eligibility restrictions, taxes or fees may apply. I would check the terms again at the fill.

How much weight people lost

Here is what each label reports from its own placebo-controlled trial in adults without diabetes:

Wegovy pill 25 mgFoundayo 17.2 mg
Average weight change−13.6% at 64 weeks−11.1% at 72 weeks
Placebo in the same trial−2.4%−2.1%

Those are different trials with different lengths and different people2,1. Nobody randomized women to one pill or the other and compared them directly. The larger numerical average in one trial does not establish that one pill is more effective than the other. I would not use a cross-trial table to promise a personal result.

Side effects

Same family, same kind of trouble: mostly your stomach. On the top Foundayo dose, 35% of people reported nausea, 25% diarrhea, 24% vomiting and 24% constipation, against 10%, 11%, 4% and 9% on placebo. Across the trials, 8% stopped Foundayo because of side effects, compared with 3% on placebo1. The Wegovy label lists a similar set of common side effects for its pill and its shot2.

Neither pill is for pregnancy. Both labels say to stop once a pregnancy is recognized1,2.

If you are on Medicare

Medicare's temporary GLP-1 Bridge covers both pills for a $50 monthly copay for people who qualify, from July 1, 2026 through December 31, 20275. Your prescriber has to confirm you meet the program's criteria, and that $50 doesn't count toward your Part D deductible or yearly cap.

Where this left me

If I were choosing a pill, I would ask which routine I can follow reliably, what the appropriate maintenance dose costs and how side effects will be handled. I would also want a contraception plan where applicable. I would not choose from one trial average alone.

If you are deciding between these, bring both names to whoever prescribes for you, and call them right away for severe stomach pain that won't ease.

Frequently asked questions

What is the difference between Foundayo and the Wegovy pill?

They are different medicines. Foundayo's active ingredient is orforglipron, and the Wegovy pill is oral semaglutide. In daily life the biggest difference is the routine: Foundayo can be taken once a day with or without food, while the Wegovy pill must be taken on an empty stomach in the morning with up to 4 ounces of water, followed by at least 30 minutes without food, drinks or other pills.

Is Foundayo cheaper than the Wegovy pill?

At the starting dose they cost the same, $149 a month self-pay. Higher up, it depends on your dose and on timing: Lilly has a temporary price cut on two Foundayo doses through December 31, 2026, and its top-dose price depends on refilling within 45 days. The Wegovy pill is $199 at 4 mg and $299 at 9 mg and 25 mg on NovoCare. Both companies have changed prices during 2026, so check before you fill.

Which pill causes more weight loss?

In their own trials, the Wegovy pill at 25 mg averaged a 13.6% weight loss at 64 weeks, and Foundayo at 17.2 mg averaged 11.1% at 72 weeks, among adults without diabetes. The trials were separate, ran different lengths and enrolled different people, so this is not a head-to-head comparison.

Does Foundayo affect birth control pills?

Its label says it can. Because Foundayo slows how fast the stomach empties, the label advises people using an oral hormonal contraceptive to switch to a non-oral method or add a barrier method for 30 days after starting and for 30 days after each dose increase. Hormonal methods that are not taken by mouth should not be affected.

Does Medicare cover Foundayo or the Wegovy pill?

Medicare's temporary GLP-1 Bridge covers both, along with the Wegovy shot and the Zepbound KwikPen, for a $50 monthly copay for eligible people from July 1, 2026 through December 31, 2027. A prescriber must confirm you meet the program's criteria, and the $50 does not count toward your Part D deductible or yearly out-of-pocket cap.
