Midi Health is still a genuine option — the most price-transparent provider I've reviewed and, at the same time, the one that asks the most of your calendar: Midi publishes every single dose and what it costs, then makes you book a 30-minute video appointment to get any of it. What we like

  • In-network with most major commercial plans and with Medicare, listed state by state, with prior-authorisation support included
  • Self-pay visit prices published openly and identically everywhere: $299 first visit, $149 follow-up
  • A dose-by-dose self-pay price list for every medication it prescribes, and it says plainly that it earns nothing on which one you pick

The caveats

  • A booked appointment is the model, with monthly check-ins while you're getting started — the least nap-friendly shape there is
  • Self-pay adds up fast: $299 for the first visit and $149 per follow-up, on top of the medication
  • Brand-only, so there is no cheap compounded floor if budget is the whole problem

What we like

  • A full dose-by-dose price list published openly, shipping and supplies included, with "No membership fees" stated on the page
  • In-network with most PPO plans, so the visit can run through insurance rather than out of pocket
  • Both molecules as injections and as sublinguals, plus brand-name GLP-1s sent to your own pharmacy

The caveats

  • A scheduled 30-minute video visit is the only way in, with the questionnaire due 24 hours early and a text confirmation required to keep the slot
  • The price climbs steeply with your dose — semaglutide more than doubles from $165.90 to $355 as you titrate
  • Visit fees are billed on top of the medication: $250 for the first and $150 for each one after, if you're self-pay
